Heart failure patient management and interventions using continuous patient monitoring outside hospitals and real world data
Retention project is a H2020 funded project conceived to support clinical decision making, clinical monitoring and data-driven interventions for heart failure patients in need of ventricular assist devices (VAD) and patients who received a heart transplant.
Heart failure patients management has a huge impact on health systems due to the required multiple and prolonged hospitalisations of patients and the related patient management costs. Focusing on routine patient-physician or patient-nurse communication following discharge can result in a significant reduction of hospitalizations.
This RETENTION-Project has received funding from the European Union’s Horizon 2020 research and innovation programme under Grant Agreement No. 965343.
